Whenever I try to accept an incoming file I click accept and the status says accepting connection but after a brief moment of time I recieve a status that says connection failed. I'm not sure as to why this is. Thank you

There are a few possibilities.
1) When you accept the file, so you have any other transfers active, via mIRC , FTP, or P2P (file sharing)?
If you do, then it's possible that your available bandwidth is insufficient to accomodate the extra transfer. Try pausing/stopping other transfers, or simply wait for one or more to end, then try again.
2) The sender has a situation similar to what I described above, in which case, there's nothing you can do about it.
3) Something else that I can't think of at the moment.

i am having the same problem. i used mirc on a home computer with no other programs open. all DCCs time out from everybody i try. what else causes this?

As has been stated there are a few possible problems, firewall, router or mirc itself each one can cause dcc to fail, sometimes it is the senders problem sometimes yours and it could also be your ISP.
Use the search feature and check the possible solutions.
