is it possible to have an input box like this

What would you like to edit?
1) Email
2) Password

the 1) and the 2) should be like checkboxes, does $input allow this?

i want to somehow call an alias if they choose to edit their email, and call another alias if they choose to edit password

$input doesn't allow checkboxes. The only thing that does allow checkboxes (literally) are dialogs, and in either case, you can't show the input query (for $input) or the dialog window (for a dialog) to someone else. Both of these items are for your own usage.
