I don't know if this is a bug or what.

I have a guy on my auto-voice list with the address *!*omar@*.du.simnet.is on the channel #max
This is what I get when I try to remove him (his nick is WB|dukE)...
Code:
/avoice -r *!*omar@*.du.simnet.is
* *!*omar@*.du.simnet.is is not in auto-voice list
/avoice -r *!*omar@*.du.simnet.is #max
* *!*omar@*.du.simnet.is is not in auto-voice list
/avoice -r WB|dukE #max 3
* *!*omar@*.du.simnet.is is not in auto-voice list
/avoice -r WB|dukE #max
* WB|dukE!*@* is not in auto-voice list

This is so weird!
I have this guy on my auto-voice list with auto-voice enabled.

Can you please help me?

Try using the -w switch for /avoice, as you haven't specified a network when adding him.

/avoice -rw *!*omar@*.du.simnet.is

I very much like this function of the /avoice (/aop /protect and /ignore too) feature. This way you dont accidently remove an entry tho it takes a bit longer to understand how it works. Simply put, if you add somebody gloablly you have to remove them globally (If you use -w when adding, you need to use -w when deleting) and if you add them "locally" you need to remove them "locally"
