I'm trying to add extra menu items to the menu you get when right-clicking on hotlinks... is this possible?

you mean when you right click on url hotlink or a hotlinks you've created?
the later doesn't have any popups (at least I've never seen one)
in both cases I don't thinks it's possible

When you right-click a link, you will see:

• Open
• Open in New Browser
• -
• Copy Address

I don't know of any way to add to that popup list, though. Neither do I know of a way to add to the popups in the editbox and other such areas.

Yes, when you put your mouse over a URL, and the 'hand' appears - then right click, a menu pops up. That menu, you cannot add to, or alter. It is a default menu.

Well what you can do is make your own popup menu for hotlinks with picture windows, ive done many.
