this was with mIRC 6.12

there are lots of threads on this, use the search feature set to "all forums" and expand the date range to "all". Start with this thread as it condenses most of what has been posted. Feel free to refer ppl who ask in your help channel to that link

wish i could tell you there is one cure fits all, but there doesnt seem to be, we can only suggest what others have tried with some success. several have reported this occurring with some some addons, so thats something to keep in mind.
