How about putting a Picture option in Options so that you can set a picture that someone could access with a /picture <nick> command?

Would you upload the picture to a remote server, making it a server specific command? Would it be sent via DCC? Or can we get the magical sqwerls (Yes, sqwerls.) to transport them from CPU to CPU?

What makes pictures so special that they need their own command? Is fserve not good enough?

ctcp *:PICTURE:?: .DCC Send $nick "C:\My Documents\My Pictures\Me in Times Square.jpg"

Put that in remotes (Alt-R), alter the filename to the picture you want, and the next time someone does /ctcp YourNick PICTURE, they'll get it straight from you. HTH.

The picture would be on the local computer, like ICQ does it. There's no need for them to be hosted on an IRC server. Sure, a person could make a script or a ctcp action for it, but incorporating it into mIRC itself would make more people aware of it and would standardize it, two critical components.
