I suggest adding an additional entry under mIRC Options... > DCC > "Auto-get file and" that reads "If file does not exist:" and then a dropdown with { Ask, Accept }.

"Ask" would allow you to approve a specific file transfer (and if you script it you can have mIRC auto retry a file transfer in the event of a timeout) while also blocking other file sends you did not approve.

"Accept" would be what mIRC does now if a file does not exist and [x] Auto-get file is selected, i.e. just allow everything in.
